足背部皮肤软组织大部份缺如

一病人因机器压伤，致足背部皮肤软组织大部分缺损约 7 * 5cm可见其伸肌腱，趾骨等均裸露，无骨折足底尚好。

请教各位前辈/同仁们。此伤如何处理为好，谢谢！！

对不起，你的用词不当，根据你的描述应该是“缺损”而不是缺如！

方法比较多，可以在 小腿做岛状皮瓣，股前外侧皮瓣，国外有做游离髂腹股沟皮瓣进行修薄后修复足部创面，外观也挺好，而且部位隐蔽。

黄埔医生您好，前几天刚从国外本杂志上看到的，当时对此也有些以外，因为这个皮瓣就是因为不易修薄而且血管变异大蔡不易用，作者的议点就是可以修薄后修复创面，而且说血供可靠。原文有空我贴上去，翻译过来也许不准确，大家讨论一下。

  On the basis of our results,the essential pedicle of groin flap is assumed to be not the superficial circumflex iliac artery itself but a dominant perforator of the surperficial or deep branch of the surperficial circumflex iliac arterysysterm,and this perforator  is responsible for the vascular territory of the flap .
   resection of a considerable amount of faaty tissue in a superfical circunflex iliac artery perforator flap is easy because a short segment (3 cm in length) of the distal portion of the superficial circumflex iliac artery systerm and the pedicleof perforator flap will hardly be damaged with renoval of  the fat ,as in the case of the groin flap .
the advantage of this flap :1 ,thinning of the flap maybe performed with primary
defatting.   2,thinning to create a customized adiposal flap with vilume reductionmaybe perforned in one stage,.........
