part two - HISTORY OF CHINESE SOCIETY AND STATE OF PRACTICE IN CHINA
In the early development of microsurgery, Chinese surgeons made tremendous contributions in the fields of limb and digit replantation, thumb reconstruction by toe transplantation, and skin flap invention. The following first – time procedures are representative of that period of clinical innovation:  the first -  reported successful distal forearm replantation was performed by Dr. Zhong- Wei Chen and coworker in Shanghai Sixth People’s Hospital in January 1963; the second toe to thumb transplantation by Yang and Gu in shang hai Huashan Hospital in February 1966; the free lower abdomen flap transfer fed by the superficial epigastric artery and vein also performed by Drs. Yang and Gu in March 1973; and the neurovascular pectoralis major (abdominal area) free muscle transfer by Dr. Zhong – Wei Chen’s group in July 1973.
In 1978, a group of orthopedic and plastic surgeons formed the Chinese Microsurgery Group, which was subsidiary to the Orthopedic Society of the Chinese Medical Association. Over the year, however, it grew and expanded, in 1989, it became the Microsurgery Society of the Chinese Medical Association. The chairmen, in this oder, were Sheng – Xiu Zhu(1989-1992, 1995-1999)from Beijing, JIa – Kai Zhu (1992-1995) from Guangzhou, Zhen – Guang Chen (1999-2003) from Wuhan, and Chun – Lin Hou (2003-2006,2006-) from Shanghai.
The first national Congress for Microsurgery of China was organized in Beijng in 1978. Since then, this scientific national congress has been held every 3 years. From September 14 to 17, 2006, the eight national congress of the Chinese Microsurgery Society was held in Qingdao, Shandong Province, and more than 500 papers were presented during the meeting. Between the national congresses, specific symposiums were organized yearly to focus on the topics of microsurgical interest, the latest of these symposiums being the second Symposium of Perforator Flaps held in October 2007 in Kunming.
In the meantime, a journal was also set up in 1978 to establish an exchange platform for its members. The journal was initially called xianwei waike, meaning Microsurgery, in 1978. In 1985, the title was changed to Xianwei yixue zazhi, meaning Journal of Micro – Medicine. Finally, in 1989 it became zhonghua xianwei waike zazhi, meaning Chinese journal of Microsurgery. This Publication has now become one of the official journals of the Chinese Medical Association. It was published quarterly from the beginning of 1978 till 2005. From2006, it was published bimonthly with each issue containing 80 pages. The Journal celebrated its 30-year birthday in 2007.
At present, clinical microsurgical practice has been widely and extensively developed throughout the country, in both university hospitals in cities and county hospitals in rural areas. Generally speaking, it involves the following areas. First, emergency microsurgery, such as replantationand direct tissue repair (especially peripheral nerve repair), is available in daily practice. Second, reconstructive Microsurgery is available throughout the country, including combined free tissue transfers, such as toes and skin flaps for hand reconstruction, bone and flaps for complicated lower limb trauma, nerves and muscle transplantation for neural defect, an so on. Third, Microsurgical treatment for specific disease is also a part of standard practice, and such Microsurgery includes bone ischemic necrosis repair (vessel or vascularized bone implant) and spinal cord injury reconstruction (nerve transfer or heterogenic neurorrhaphy). Fourth, microsurgery is combined with other surgical techniques, such as tissue engineering, flap prefabrication, endoscopically assisted, and video Microsurgery. Fifth, microsurgical technique is used in basic clinical and reseach study, sush as anatomic perforator flap and compound flap investigations. With improvement of facilities, microsurgical animal experimental studies are also well represented in many university laboratories.
China is a rapidly developing country with very large population. Trauma and its sequelae are the main aspect of our Microsurgery practice. With improved financial status, English fluency, and the collaboration of Microsurgery, we can predict that, in the future, the Chinese Society of Microsurgery will bring more informative data to the world, and that, more than ever before, Chinese achievements in Microsurgery will appear in the international limelight.
