【解剖概要】
臂丛由C5～T1神经根组成。其中C5～6神经根形成上干，C7神经根形成中干，C8～T1神经根形成下干。每干移行到锁骨中1／3后时，各分出前后两股。其中三个后股组成后侧束，上、中干前股组成外侧束，下干前股形成内侧束。臂丛后侧束发出腋神经、桡神经，外侧束发出肌皮神经和正中神经外侧头，内侧束发生尺神经、正中神经内侧头、前臂及臂内侧皮神经。
【诊断】(按下列程序进行)
1．判断有无臂丛神经损伤：若上肢五大神经中任何两大神经的联合损伤(非切割伤)，表明有臂丛损伤。
2．判断损伤部位：根据背阔肌、胸大肌、胸肋部是否有肌萎缩，可判断损伤部位是在锁骨上或锁骨下。若肌肉萎缩，则表示损伤部位在锁骨上。
3．臂丛神经根、干、束、支的定位诊断：根据上肢五大神经损伤后肌萎缩出现，进行组合判断。
4．是否有根性撕脱伤：根据症状、体征相结合一般可判断。其中，Tinel征阴性提示节前损伤，Horner综合征提示下干根性撕脱伤，肩胛提肌、斜方肌麻痹提示上干根性撕脱伤。
【治疗】
若臂丛损伤后3～6个月内肌力逐渐恢复，可继续观察。若在此期间毫无恢复，则考虑手术探查。若非根性撕脱伤，更宜早期探查进行神经松解术或神经移植、缝合术。对待根性撕脱伤，可考虑行膈神经、副神经、肋间神经及颈丛神经移位术，功能可获一定程度的恢复。对于晚期或根部臂丛损伤无法行手术修复者，可按残存肌肉情况做肌腱移位或关节融合术。 不过做过损伤后修复手术的病历也不多，估计还是很有难度的。
